Position Summary

​

The Hospice Case Manager provides compassionate, patient‑centered nursing care to individuals receiving end‑of‑life services in their homes or residential settings. This field‑based role is responsible for coordinating the full spectrum of hospice care, managing a caseload of patients, and ensuring that each individual and family receives exceptional clinical, emotional, and educational support.

The Case Manager serves as the primary point of contact for patients, families, and interdisciplinary team members, ensuring continuity, quality, and dignity throughout the hospice journey.

Key Responsibilities

​

Patient Care & Clinical Management

  • Provide skilled nursing assessments, symptom management, and end‑of‑life care in patients’ homes

  • Develop, implement, and update individualized care plans based on patient needs and goals

  • Monitor changes in condition and adjust interventions accordingly

  • Administer medications, treatments, and comfort‑focused interventions

  • Educate patients and families on disease progression, symptom control, and hospice philosophy

  • Ensure timely documentation in accordance with regulatory and organizational standards

Case Management & Coordination

  • Manage a caseload of hospice patients across a defined geographic area

  • Serve as the primary nurse coordinating care with physicians, social workers, chaplains, aides, and volunteers

  • Facilitate communication between the patient, family, and interdisciplinary team

  • Arrange and oversee DME, supplies, and supportive services

  • Participate in interdisciplinary team meetings and case conferences

Family Support & Communication

  • Provide emotional support, anticipatory guidance, and education to families and caregivers

  • Assist families in navigating end‑of‑life decisions and available resources

  • Respond promptly to patient and family concerns, ensuring compassionate, timely follow‑up

On‑Call & Emergency Response

  • Participate in rotating on‑call coverage as assigned

  • Respond to urgent patient needs, symptom crises, and after‑hours concerns
     

Qualifications
 

  • Current RN license in the state of practice

  • Minimum 1–2 years of nursing experience; hospice, palliative care, home health, or critical care preferred

  • Strong clinical assessment and symptom‑management skills

  • Valid driver’s license and reliable transportation for daily field visits

  • Excellent communication, empathy, and patient‑family engagement skills

  • Ability to work independently, manage time effectively, and prioritize patient needs

  • Comfort with electronic documentation and mobile charting
     

Preferred Attributes
 

  • Certification in Hospice & Palliative Nursing (CHPN) a plus

  • Experience managing a community‑based caseload

  • Strong understanding of end‑of‑life care principles and comfort‑focused interventions
