In Home Hospice/Palliative Care Physician

Position Overview

​

We are seeking a compassionate and skilled Hospital-Based Physician to provide home-based palliative and hospice care for patients transitioning from the hospital to their residences. This physician will serve as a vital link between inpatient teams and community-based care, ensuring continuity, comfort, and dignity for patients and families during advanced illness and end-of-life care.

Key Responsibilities

​

  • Clinical Care

    • Conduct home visits to provide medical management for patients requiring palliative or hospice services.

    • Assess and manage symptoms including pain, dyspnea, nausea, anxiety, and other complex needs.

    • Coordinate medication management, durable medical equipment, and supportive therapies in the home setting.

  • Continuity of Care

    • Collaborate with hospitalists, specialists, nursing staff, and case managers to ensure smooth transitions from hospital to home.

    • Maintain ongoing communication with hospice nurses, social workers, chaplains, and caregivers to deliver holistic care.

  • Patient & Family Support

    • Provide compassionate education and counseling to patients and families regarding prognosis, goals of care, and treatment options.

    • Support families in navigating emotional, spiritual, and practical aspects of end-of-life care.

  • Documentation & Compliance

    • Complete accurate and timely documentation in the hospital’s electronic health record and hospice systems.

    • Ensure compliance with hospital policies, hospice regulations, and accreditation standards.

 

Qualifications

​

  • MD or DO degree with board certification/eligibility in Internal Medicine, Family Medicine, or Palliative Medicine.

  • Current, unrestricted medical license in the state of practice.

  • Experience in palliative care, hospice, or geriatrics preferred.

  • Strong communication, empathy, and critical thinking skills.

  • Ability to work independently in home settings while collaborating with hospital and hospice teams.

  • Valid driver’s license and reliable transportation for home visits.

 

Work Environment

  • Hospital-based role with regular travel to patient homes within the service area.

  • Flexible scheduling with potential evening or weekend responsibilities.

  • Integration with both inpatient and community-based hospice/palliative care teams.
