Inpatient Palliative Care Pharmacist

About the Role

​

We are seeking a dedicated and compassionate hospital Clinical Pharmacist specializing in palliative care to join the interdisciplinary team. This position provides expert medication management, symptom relief, patient and family support, and clinical education while ensuring compliance with hospital standards and codes of practice.

As a Clinical Pharmacist, you will play a vital role in evaluating therapies, guiding safe and effective medication use, mentoring clinicians and trainees, and advancing best practices in palliative care pharmacotherapy.

 

Key Responsibilities

​

Patient Care & Clinical Expertise

  • Provide palliative care for hospitalized patients and families facing serious medical problems.

  • Conduct comprehensive clinical evaluations, including medication histories, disease state assessments, and patient/family needs.

  • Review medications for appropriateness, drug-disease interactions, and adverse drug events.

  • Order, interpret, and evaluate diagnostics and laboratory data, including pharmacogenetic testing.

  • Collaborate with the care team to establish patient-centered goals of care.

  • Recommend and monitor therapies for pain and symptom management.

  • Educate patients and families on pharmacologic and non-pharmacologic interventions.

  • Document patient care activities in accordance with hospital standards.

 

Safety & Risk Management

  • Implement risk mitigation strategies to optimize safe and effective medication use.

  • Ensure safe handling of high-risk medications, including dosing, administration, and disposal.

  • Provide recommendations for nonstandard dosage forms and routes of administration.

  • Address financial aspects of medication use by considering hospital, hospice, and insurance formularies.

 

Education & Collaboration

  • Provide mentorship and cross-training for clinicians and trainees in pharmacotherapeutic concepts.

  • Receive cross-training from other disciplines to enhance interdisciplinary collaboration.

  • Participate in the development of protocols and clinical pathways to improve palliative care.

  • Engage in interprofessional education through both didactic and experiential learning.

  • Represent the team through presentations at local, regional, and national forums.

  • Serve on committees and work groups to advance hospital initiatives.

 

Administration & Compliance

  • Review and evaluate medication orders by authorized practitioners and dispense medications per procedure.

  • Maintain accurate inpatient records (manual and electronic).

  • Prepare departmental reports as assigned.

  • Adhere to hospital policies, procedures, and quality/safety standards.

 

Qualifications

​

  • Active New York State pharmacy license in good standing

  • Two years of post-graduate residency/fellowship training or equivalent experience in oncology, hospice, geriatrics, or related fields preferred

  • Board certification or other clinically relevant certification preferred (e.g., Board of Pharmacy Specialties, American Society of Pain Educators)

  • Strong expertise in medication management, symptom relief, and interdisciplinary collaboration

  • Commitment to compassionate, patient-centered care and continuous professional development
