Position Overview

We are seeking a dedicated Wound, Ostomy, and Continence Nurse (WOCN) Registered Nurse to join our hospital’s hospice care team. This specialized role provides advanced wound, ostomy, and continence management for patients receiving end-of-life care within the hospital setting. The WOCN RN ensures comfort, dignity, and quality of life by integrating clinical expertise with compassionate support for patients and families.
Key Responsibilities

Clinical Care

Conduct comprehensive assessments and deliver individualized wound, ostomy, and continence care for hospice patients admitted to the hospital.

Implement evidence-based wound care interventions, including advanced dressings, infection prevention, and pain management strategies.

Provide ostomy education and continence support to patients, families, and caregivers in the hospital environment.
Interdisciplinary Collaboration

Partner with physicians, hospital nurses, palliative care specialists, social workers, chaplains, and allied health professionals to develop holistic care plans.

Serve as a clinical resource for hospital staff regarding wound and ostomy best practices in hospice care.
Patient & Family Support

Offer compassionate education and emotional support to patients and families navigating complex care needs in the hospital setting.

Promote dignity and comfort through proactive symptom management and clear communication
Documentation & Compliance

Maintain accurate, timely documentation in the hospital’s electronic health record system.

Ensure compliance with hospital infection control standards, hospice regulations, and accreditation requirements.
Qualifications

Current Registered Nurse (RN) license in the state of practice.

Certification as a Wound, Ostomy, and Continence Nurse (WOCN) required.

Minimum of 2 years of nursing experience; hospital, hospice, or palliative care experience preferred.

Strong clinical assessment, critical thinking, and communication skills.

Ability to work effectively in a fast-paced hospital environment while maintaining a patient-centered focus.
